Gayaneh Kostikyan

About Gayaneh

Gayaneh Kostikyan comes from a background in public health and epidemiology.  She has worked nearly 10 years as a clinical consultant for large health plans in Southern California.  In her consulting role, she managed the clinical and quality performance of several of the largest IPA's and medical groups in Central and Southern California.  With her knowledge of applied biostatistics, she brings skills in identifying and addressing areas of opportunity in under-served communities.

About Dr. Badday

Dr. Badday is a skilled physician specializing in internal medicine.  He received his Medical Degree from the University of Baghdad, Iraq. After completing his internship in 1982 and residency in 1984 in Internal Medicine at Cook County Hospital in Chicago, he relocated his family to Southern California.  He holds Medical Staff Membership at Pomona Valley Hospital Medical Center (PVHMC) in Pomona, Ca and San Antonio Reginal Hospital (SARH) in Upland, Ca. He was formerly the Chief of Staff at PVHMC and Medical Department Chair at SARH.

 

Dr. Badday is known for providing personalized, evidence-based care to his patients.

When we make a decision to approve or deny your care, this is called Utilization Management (UM).  CHP wants you to know that decisions about your health care are based on:

  • If the care and/or service is right for your condition 

  • If your benefits cover the care and /or service

 

Our doctors and other health care staff do not receive money or other rewards when making decisions about your care.  CHP does not receive money or other rewards when making decisions about your care. We do not reward staff to make decisions that result in care that is less than what is being requested.

Availability of Criteria

Capital Health Partners makes available a copy of the criteria or guidelines used in making a medical decision to its providers.  A copy of the criteria or guidelines are also made available  to the public and members upon request.  Reasonable fees may be applied to cover administrative expenses such as copying and postage costs.
